I hereby request you to change the mobile number and / or email address in Sampath Vishwa online banking system as per the above mentioned mobile phone number and / or email address and hereby agree to be bound by the following terms and conditions. 1. If the mobile phone is stolen or lost, I shall notify the bank immediately in writing. 2. If the mobile number is changed or transferred, I shall notify the bank immediately in writing. 3. If the email address is compromised or becomes inaccessible to me, I shall notify the bank immediately in writing. Further I agree to indemnify and hold harmless Sampath Bank Plc from any damages, claims, demands, proceedings, losses, costs, charges and expenses whatsoever including legal charges and attorney’s fees which Sampath Bank Plc may at any time incur, sustain, suffer or be subjected to as a consequence of, or arising out of, any of the following: (I) my failing to inform you immediately upon the occurrence of any of the above stated events; and/or (ii) my providing an incorrect number or a number that belongs to a third party or incorrect email address or an email address that belongs to a third party (III) my sending any instruction through the above mobile number and / or email address. I shall not hold Sampath Bank Plc liable for any loss, damages, claim, expense including legal costs that may be incurred/suffered by me on account of the SMS / email / alert / push notification facility.
